The Occupational Safety and Health Administration, or OSHA, will be the investigating agency.

Matthew D. Loeb, president of the International Alliance of Stage Employees, said the union community was shocked and saddened.

“We are working to provide support to our member's family, colleagues and co-workers,” Loeb said in a statement. “Workplace safety is our top priority, and we will assist Cal/OSHA in their investigation to the best of our ability.”

Although crew injuries and deaths in film and television have historically been underreported, several deaths have occurred in recent years, prompting high-profile lawsuits and calls for industry reform.

These include the 2014 death of 27-year-old camera assistant Sarah Jones, when a gun pointed at actor Alec Baldwin went off on the set of cinematographer Halina Hutchins' film Rust in 2021. Killed in a train accident on the set of Midnight Rider.
